A 62 year old diabetic female presented to the outpatient unit with complaints of chest discomfort in the night, which had settled in the morning. The serial ECGs showed clear signs of “Wellen’s syndrome”. As shown in the figure, both Type A Wellen’s– biphasic T wave inversions and Type B Wellen’s– deep T wave inversions were noted. This finding is important to pick up, as it suggests significant occlusion of the proximal LAD coronary vessel. This patient immediately had an angiogram followed by angioplasty to the vessel.
